Our Approach
Our individualized approach to meeting the needs of residents and their families includes:
- 24/7 nursing staff
- Nursing manager supervision
- Full-time social worker
- Day and evening activities staff
- Family support group
Our activities are carefully designed for residents with both short- and long-term memory impairments, and include exercise, music, crafts, cooking programs, and much more. We also have a dementia care specialist on staff.
24-Hour Long-Term Skilled Nursing
Our experienced nursing team is made up of more than 85 registered nurses, licensed practical nurses, and nursing assistants. Registered nurses and licensed practical nurses are on duty 24 hours a day, along with state-certified nursing assistants who further enhance the care that each resident receives.
Residents may use their own primary care doctor, one of our medical directors, or choose from more than 18 physicians credentialed to conduct on-site visits. Telehealth is also available to treat residents after hours/weekend. Dental, podiatry, vision, audiology, diagnostic services, lab tests, blood glucose monitoring, wound care,portable x-rays, and IV therapy are just some of the medical services provided. Social workers are also available to offer guidance should care needs evolve.
Our high nurse-to-resident ratio, daily health assessments, and a wide variety of wellness services foster an environment of resident-centered care that makes all the difference.
Short-Term Rehabilitation Services
The Bridge at Elan
Our transitional care unit provides both rehabilitation programs for seniors with conditions such as knee or hip replacements and recovery following traumatic injuries, as well as high-acuity nursing care and clinical management for medically complex patients transitioning from the hospital. We provide a bridge between your hospital stay and home. Our accelerated rehabilitation programs and enhanced clinical capabilities are designed to assist you with getting back to your life. Our newly renovated unit offers a well- appointed environment and amenities to enhance your healing.
Our short-stay rehab program provides in-house therapies within 24 hours of admission and up to six days a week. We support recovering individuals through in-patient rehabilitation and assist long-term care residents with chronic or special needs. As an alternative to hospital-based rehabilitation care, our short-term rehabilitation program serves conditions such as knee or hip replacements and recovery following traumatic injuries, and can bridge the gap between your hospital stay and home.
Discharge planning meetings are held by the staff on a routine basis. In reviewing the plan of care and discharge needs, we work to determine what is truly important to the patient, family, and caregivers with the goal of getting you back home where the best healing happens.
We partner with HealthPro Heritage to provide skilled physical, occupational, and speech therapy to assist our short-stay patients in developing increased levels of strength, mobility, and independence in an on-site private therapy gym. We design a custom plan to help you achieve your goals.
Founded as the Jewish Home of Eastern Pennsylvania, Elan Skilled Nursing and Rehab has grown to become a leading provider of short-term rehabilitation and long-term skilled nursing care.
Future Plans
In early 2023, we will open our newly renovated second floor, which includes a 30-bed, state-of-the-art, short-term rehabilitation unit. This unit will position us to better serve the post-acute population that needs transitional care between the hospital and home. We also assist in providing therapy to long-term residents whom will be residing here and aging in place. Some programs, like our Ageless Abilities, include a focus on caregiver/family training for persons with dementia.

Our Personalized Approach to Rehab
Therapy, nursing, and social services professionals work as a team to tailor your care to fit your individual needs. Your discharge plan starts from admission. We work with you to accomplish your goals. We promote education and encourage self-management to achieve and maintain the highest level of function.
- Our dietary department serves healthy homemade meals prepared in our kitchen.
- Our full-time therapy staff includes physical therapists, occupational therapists, and speech therapists.
- We have a full rehab gym to meet your needs as well as CPR-trained licensed nurses.
Well-Equipped Therapy Department
We have built our reputation on years of clinical excellence, and are proud to offer a variety of rehabilitative services that can greatly enhance independence. Our modern, well-equipped therapy department, staffed by licensed physical, occupational, and speech therapists, offers:
- Power trainer machines for non-impact arm and leg exercise
- Parallel bars and stairs to improve gait and balance
- Ultrasound for pain, mobility, and wound healing
- Diathermy, moist heat, ultrasound, and electric stimulation to relieve pain and improve mobility
- Short-term programs for transitioning from a hospital stay, staying safe at home, and other such needs
We also offer a restorative nursing program, featuring unique protocols tailored to each resident’s needs, improving and maintaining ambulatory skills, building core strength, and fostering greater mobility.
On-Site Dialysis Services
A 1,200-square-foot section of our newly renovated second floor includes a dialysis den, where we will partner with DaVita Kidney Care, an international leader in renal care.
Our partnership with DaVita will bring their qualified nephrologists and dialysis team directly to our residents. In early 2023, we expect our first residents to receive treatment with the first inpatient dialysis offered in a skilled nursing facility within Northeastern Pennsylvania.
The dialysis den will enhance quality of life for both our short-term and long-term residents, who will no longer have to travel outside for their dialysis treatments. Providing dialysis on-site is not only a more cost-effective treatment option, but also provides higher-quality outcomes; better coordination of care; less chance of hospital readmission; and less risk of infection. Our residents will spend less time pursuing their dialysis treatments and more time on therapies and social activities. Because the care & rehab team will be on-site, the dialysis team will work closely with the Elan Skilled Nursing team to care for the resident holistically, embracing all of the resident’s needs in their treatment plan.
Telemedicine
Elan Skilled Nursing & Rehab is dedicated to fostering our residents’ well-being. Through Third Eye Health telemedicine and a highly skilled medical team, we can connect you with a physician anytime during after-hours. You can discuss concerns and treatment options with a doctor who is geriatric trained and specializes in post-acute care.
What is telemedicine?
- Live and interactive communication between you, a nurse, and a doctor, through the use of secure video and audio at your bedside
- With the help of a nurse, the doctor uses a portable system that contains audio, video, and diagnostic devices to see, talk to, and examine you remotely.
What are the benefits?
- Quick treatment from the Third Eye Health doctor, potentially avoiding a transfer to the hospital or doctor’s office
- Evaluation from a doctor who treats you in the comfort of your surroundings
Privacy and security of your personal health information is the top priority at Third Eye Health. Your visit will not be recorded. Your health records will be stored according to federal HIPAA regulations and shared with the Elan Skilled Nursing team after a consult.
How it works
At Elan Skilled Nursing, we use Third Eye Health technology, which is a portable medical system that can be brought right to your room to virtually connect you with a Third Eye Health doctor to evaluate, diagnose, and treat you. Our three-step process:
- The nurse recognizes you are in need.
- The nurse calls Third Eye Health or schedules a consult.
- You, the nurse, and your doctor discuss your concerns.

We have partnered with Third Eye Health to provide the best skilled nursing care after-hours. We allow their team of specialized doctors to see, hear, and speak to our residents through secure telemedicine technology. This allows our residents to be treated by doctors who are trained in geriatric care, so residents can stay in the comfort of their own residence – potentially avoiding an unnecessary trip to the hospital. Third Eye Health specializes in post-acute care, with an over 94% treat-in-place rate.
